Filing a claim form for VSP is necessary when you us an in-network provider and you did not confirm your VSP coverage at the time you made your appointment. You will need to complete the VSP Out-Of Network Reimbursement Form. Send the completed form with related receipts to the address noted on the bottom of the reimbursement form. All claims need to be mailed within six (6) months of the date of service.
