If you receive emergency care from a non-Kaiser Permanente physician or medical facility, you will need to complete a Claim for Emergency Medical Services Form. Complete both sides of the form. Attach applicable documentation. Make a copy of the Claim for Emergency Medical Services form for your records. Date and sign the form. Mail your completed form, along with any bills to one of the appropriate address on the form.

Mail Order Prescriptions

First-time Prescription Orders: Obtain a written prescription from your physician and have your initial order filled at a Kaiser Pharmacy.

Ordering Refill Mail Order Prescriptions: Your can refill your mail order prescription in one of three easy ways:

Select your region and register
You’ll receive your password in the mail in 3-7days
To order refills, just clink on “Appointments/RX refills.”

Call 1-866-206-2981 toll free, or the phone number at the top of your prescription label.

Use the Prescription Refill Mail-Order Request Form.

Complete Member information; use one box per member, if you need more room, attach a separate piece of paper.
All orders must be paid in advance (paying by check may delay your refill by up to 5 days. Do not send cash.
Your refill will be mailed to you at no additional charge. Please allow 7 to 10 days for your order to processed.
