 Health Care Reimbursement Claims 

 
 

 

 

To file a claim for your Health Care Reimbursement Account, you will need to complete the WageWorks Pay Me Back Claim Form.  You can fax the completed claim form to the fax number provided on the form or mail the form to the address on the form.  Send the signed form along with receipts for each expense incurred.  Your receipts must include the date(s) of service.  Do not send original receipts; save them for the IRS.  Attaching multiple receipt(s) is okay, as long as you circle or check the dollar amount that is being claimed for each receipt. Do not use a highlighter.


All 2010 eligible Health Care expenses must be incurred by March 15, 2011 and claims be filed no later than May 31, 2011.  If filing by mail, it must be post marked no later than May 31, 2011.
