To file a claim for your Dependent Care Reimbursement Account, you will need to complete the WageWorks Pay Me Back Claim Form. You can fax your completed claim form to the fax number provided on the form or mail the form to the address on the form. Send the signed form along with appropriate proof of service for each of your expenses or have your service provider sign the form.
All 2010 eligible Dependent Care expenses must be incurred by March 15, 2011 and claims be filed no later than May 31, 2011. If filing by mail, it must be post marked no later than May 31, 2011.
